To ensure proper recordkeeping, please print, complete, sign, and return these forms to Miss Melissa’s Preschool in Kelowna. As a licensed facility, we require all forms to be completed before your child attends classes.

 

We also ask all parents or caretakers to read through our Parent Handbook to learn about important policies and procedures.
